Terms and Conditions for New Patient Promotion offer

  • Exclusive promotion for new patients of Bare Teeth Dental.

  • To redeem promotion, the patient must mention the promotion when booking the appointment.

  • Promotion is valid for item numbers 011 (comprehensive oral examination), 022 (dental x-ray), 072 (Photographic Records - Intraoral), 073 (Photographic Records - Extraoral), 114 (removal of calculus), 121 (fluoride treatment)

  • Services will only be provided when suitable and appropriate.

  • Health fund card must be presented at the appointment to claim the health fund rebate.

  • Health fund rebate maybe greater than $199. If the health fund rebate is greater than the discounted amount, any excess amount will not be credited or refunded.

  • Health fund rebate is subject to payment by the health fund and the health fund terms and conditions including annual limits and eligibility. the patient will be responsible for payment of $199 if payment is not made by the health fund. Check with your health fund prior to your appointment.

Additional Terms and Conditions When Using Private Health Fund

Bare Teeth Dental has no control over how much a private health fund pays towards a dental treatment. The amount of rebate towards a treatment is entirely between patients and their health fund. It is patients’ responsibility to contact their health fund regarding its terms and conditions, especially the benefits the health fund provides and limits and restrictions on treatments.

Bare Teeth Dental does not accept responsibility when the health fund fails to pay the benefits. Patient is responsible for paying the account whether the health fund contributes to or not.

With your permission, we can process your health fund card through our HICAPS terminal as a complimentary service for your convenience.

Additional Terms and Conditions When using other Third Party Schemes.

When using third party dental schemes such as the Child Dental Benefits Schedule (CDBS), we must abide by the individual terms and conditions set out by the scheme. It is the patient’s responsibility to contact the third party dental scheme of their choice regarding to terms and conditions, especially the benefits provided and limits and restrictions on use.

Bare Teeth Dental does not accept responsibility when the scheme fails to pay the benefits. Patient is responsible for paying the account whether the scheme contributes to or not.
